CombiMatrix is committed to equity of patient care. We feel that all patients should have access to physician-recommended diagnostic testing, regardless of insurance status. To this end, we have created a billing policy that reflects our commitment to provide unparalleled diagnostic services at a responsible cost.
How does it work?
For insured patients, no upfront payment or credit card number is required prior to testing. Once the results of the test have been finalized, we will bill the patient’s insurance using the insurance information provided on the Test Requisition Form.
Please note that if you are using insurance to cover the cost of your testing, you will receive an “Explanation of Benefits" (EOB) from your insurance provider. THIS IS NOT A BILL. Although an EOB can look like a bill, it will typically have the words “this is not a bill” printed on it. If you have any questions or concerns after receiving your EOB from your insurance provider, we encourage you to contact our Billing Department at 800.710.0624 option 2 (9:00am to 5:00pm CST, Monday-Friday).
Our Billing Department works directly with your insurance company to secure the maximum allowable reimbursement, thereby minimizing any out-of-pocket costs to you. We submit the initial insurance claim and, if necessary, we will attempt multiple appeals. After such efforts, a patient bill may be issued. In the rare case of an insurance coverage denial, we encourage you to proactively work with our Billing Department.
Upon receipt of any payment made directly by your insurance provider for a test we performed, you will need to remit payment to us immediately.
Endorse the check by writing “Pay to the order of Invitae” and sign your name below. If you prefer, you may remit payment by sending a personal check or money order. Send to:
